
Reporting to the Programs Director, the Resource & Outreach Specialist is responsible for case management services and support to zero8hundred program participants. Additionally, the Resource & Outreach Specialist is responsible for community outreach to encourage and facilitate program enrollment. Further, the Resource & Outreach Specialist is responsible for resource partner outreach and engagement to develop a robust understanding of the community services and opportunities available to program participants so as to be able to refer them to those uniquely best suited to their individual transition needs. The Resource & Outreach Specialist will provide support to transitioning active duty service members, reservists, national Guardsmen and recent veterans (who separated within a year from active duty service) and their spouses through telephone/email and in-person meetings to assist them on their journey from military service to civilian life. Areas of referral partner support include post-service employment preparation and placement, educational opportunities, health and wellness services, basic needs and social/community connections. zero8hundred provides supportive counseling and coaching services, while referring program participants to their local area vetted service providers and opportunities in these critical areas. Follow-up support from the Resource & Outreach Specialist ensures services are delivered in an effective and coordinated fashion.
This position also has administrative support responsibilities as outlined below. Key Duties and Responsibilities Case Management: • Build rapport with transitioning active duty service members, veterans, reservists, national Guardsmen and their spouses who have enrolled in the zero8hundred program. • Assist zero8hundred program participants with identifying the service providers and opportunities uniquely best suited to their transition needs in the local area where they plan to live, work, go to school or drill post military service. These service providers may include resources for employment, education, housing, mental and physical health and well-being, legal services, financial support, social/community connections or any other critical area of support that may be required. • Refer program participants to zero8hundred approved service providers, in their area of need and within their local area, and monitor their progress towards key outcomes. • Remain connected with zero8hundred program participants, throughout their enrollment period, conducting follow-up calls, emails and office meetings as required to enable all aspects of the participant’s transition plan to be executed. • Continue to check in frequently (following zero8hundred’s protocol, and program participant’s request) to ensure their well-being and are making progress towards their transition goals. • Record, update and maintain accurately, and in a timely manner, all client data and outcomes in the zero8hundred case management database and other tracking software for follow-up and reporting. • Regularly audit and update data of assigned program participants. • Consult with the Programs Director when clinical concerns arise beyond the scope of training. Community Outreach and Resource Navigation: • Work professionally and knowledgeably with eligible program participants with the goal of enrolling those qualified in the zero8hundred program. • Represent zero8hundred at on and off-base venues and make presentations about the zero8hundred program at transition workshops, fairs and classes. • Conduct outreach to military commands to encourage the dissemination of information about the zero8hundred program to their transitioning personnel and facilitate those personnel enrolling in the zero8hundred program. • Identify, cultivate and maintain effective working relationships with representatives from a wide variety of resource providers, partner organizations and other stakeholders. • Represent zero8hundred at partner and collaborative meetings. • Research new potential resource providers and update and maintain correct and detailed information regarding current resource providers so as to develop a robust database for targeted referral. • Continue to learn about issues, resources and opportunities related to transitioning service members, veterans and their families. • Support the Programs Director and CEO with day-to-day program operations and administration as assigned. • Participate in internal and external committees and staff meetings as assigned. • Encourage open expression of ideas and opinions for staff development and zero8hundred program improvement. • Work responsibly in what will be a semi-independent work environment; communicate regularly with Programs Director and submit progress updates as directed by Programs Director. • Observe confidentiality, privacy, and dignity of each program participant and zero8hundred team member. • Stay current with and adhere to all requirements established by the California Board of Behavioral Sciences (BBS) and National Association of Social Workers (NASW) or the American Association of Marriage and Family Therapists (AAMFT) Codes of Ethics
